Seminar 1: Case study Emma Granström From Complex interventions in health I chose two examples of difficult change in implementation interventions. The examples of implementation interventions are in the form of randomized controlled trials to improve health care workers use of hand hygiene (p. 269 in Richards and Rahm Hallberg, 2015). Both implementation interventions were carefully planned and both used theory. The first example consisted of three different ways of using feedback and was based on goal and control theory. The intervention was marginally effective but the reality of busy health care units made evaluation hard and the team experienced lack of data for evaluating the process as well as units dropping out of the intervention. The second example consisted of plural intervention components and levels, and the implementation focused on teams and team leaders. It was based on social influence theory and contained reminders, education and feedback. This implementation intervention was more complex but also more successful in terms of effectiveness. The explanation provided in the book is that the second example took the complex context of real-life health care, with its time and resource limits and organizational priorities, into consideration in planning and executing the implementation which made it more successful. In order to further elaborate on potential enablers and barriers in the two examples, perhaps not mentioned in the cases as described in the book, I will use the PARIHS framework for implementing research into practice. The framework consists of evidence, context and facilitation (referred to on p. 187 in Greengalgh et al., 2005). Evidence consists of research evidence, clinician experience and patient experience. Context consists of organizational antecedents, organizational culture, leadership and evaluation/feedback – all with sub-categories.
